How to Request Account Deletion
Email Request (Recommended)
Send an email to: care@physiohub.in
Subject Line: Delete Phyzio Hub Account
Please include the following details in your email:
- Full name
- Registered email address
- Registered mobile number (if any)
- Account type: Patient / Practitioner
- Reason for deletion (optional)
* We will verify your identity before processing the request to ensure account security.
You can also request deletion from the app: open Phyzio Hub Patient or Doctor app → Profile → Account Settings → Delete account, then confirm with OTP.
What Gets Deleted
Data removed or anonymized upon approval
Profile Information
Name, email, phone number, photo, addresses, and personal details
Login Credentials
Authentication tokens, OTP sessions, and device / push tokens
Bookings & Care Data
Upcoming bookings and in-app care activity linked to your profile
Messages & Activity
Chat messages, notifications, and activity history associated with your account
What May Be Retained
Legal & Audit Retention Terms
Some information may be kept for a limited period if required by law, fraud prevention, payment/audit records, GST invoices, or dispute resolution. Retained data is stored securely and only for as long as needed. Clinical notes required for lawful healthcare record-keeping may be retained in anonymized or restricted form and are never used for marketing.
Typical Retention Period
Up to 30 days for backup and processing, unless a longer legal retention period applies (e.g. payment records, typically up to 7 years).

Acknowledgement
Within 2–3 business days
Deletion Completion
Within 7–14 business days after verification
You will receive a confirmation email once deletion is complete.
Important Notes
- Account deletion is permanent and cannot be undone.
- You will lose access to Phyzio Hub Patient and Doctor services linked to that account.
- Any remaining wallet balance cannot be used after deletion. Mention an active subscription or pending payout in your request if applicable.
